Dry needling therapy is a form of Physical therapy used to treat myofascial (muscle related) pain. During the therapy, a dry needle (without any injection or medication) is inserted through the skin into the areas of the muscle known as trigger points.
A trigger point is a tightened band of skeletal muscle within a larger muscle group. Trigger points can be sore to touch and applying any sort of stimulation on the trigger point may result in pain in other parts of the body. The relation between the trigger points and the related muscle group is what helps the therapy of dry needling. This is also the reason why this form of therapy is often referred to as ‘trigger point dry needling’ or Intramuscular manual therapy. This therapy is often used to treat muscle pain, even fibromyalgia pain, and sports injuries.
It is often confused with Acupuncture, a traditional Chinese medical technique performed by Acupuncturists. The two practices are different and dry needling is based on principles of Modern Western Medicine and supported by research.
Techniques
- In & Out Techniques
Pistoning or Sparrow pecking techniques are sometimes used by some therapists. Both these techniques are based on in-out insertion of the needle. The needles are used to prick the trigger point instead of being left in there for an extended time period.
- Time-based Techniques
In this technique, the needle is inserted to the trigger point and left there for a time period, usually 30 minutes. It is believed that doing so relaxed the muscle and helps release the knots that are causing pain. The duration of which the needle is left in the muscle is based on individual requirements as ascertained by his/her physical therapist.
- Non-trigger point techniques
Some therapists are of the school of thought that believes dry needling addresses a broader arena of the central nervous system. In this technique, the dry needle instead of being inserted in the area of pain- it is inserted in areas around the point of pain. The ideology behind this being that pain results from the greater nerve or muscle tissue instead of the painful muscle point being the isolated cause of pain.

Dry needling is a form of Physical therapy that is in its early stages. Hence there are no established forms of therapy, thereby it has no regulatory agencies to monitor training, licensing or supervision of therapists and their techniques. Lack of any credentialing board makes it difficult to ascertain the legitimacy of the training of a physical therapist. This is one of the biggest risks involved.
